Job Details

The Case Manager supports social assistance participants using a strong emphasis on empathetic, person-centered, strengths-based support. The role goes beyond administering benefits and determining ongoing eligibility for Ontario Works. The Case Manager works collaboratively with individuals to reduce barriers, support stability, and help people move toward their own goals by conducting comprehensive assessments, developing action plans, and coaching conversations while providing wrap-around supports. The Case Manager completes the second part of the Ontario Works application process in accordance with the provincial risk-based approach, which includes the completion of appropriate forms, and accurately applying provincial legislation, directives, and local policies.The Case Manager assesses the need for other programs and services including, but not limited to, Housing and Homelessness programs, Children’s Services, Food support, and various community resources. Working within an integrated human services model, the Case Manager provides systems navigation, outreach, referrals, conflict resolution, housing stabilization supports, homelessness assessments, and safety/risk management. The position liaises with shelters, landlords, community agencies, and service partners to coordinate supports, while maintaining thorough documentation, performing administrative tasks, managing financial entitlements, and supporting legal processes such as spousal support and deductions. The Case Manager also contributes to emergency management functions and assists in training new team members.

Qualifications

The ideal candidate will have a minimum two‑year Community College diploma in a Human Services–related program such as Social Service Worker or Mental Health and Addictions Worker. A related university degree (e.g., Social Work, Psychology) is preferred, and eligibility for registration with the Ontario College of Social Workers and Social Service Workers is considered an asset. The role requires at least two years of directly related experience in the social services field in a coaching or counselling capacity, with emphasis in areas such as Social Assistance, Employment Counselling, Addictions and Mental Health, Crisis Response, Equity and Diversity, and Housing‑Based Case Management. Candidates should demonstrate experience applying solution focused approaches, Trauma‑Informed Care, and Strengths‑Based approaches in client‑centred work.

Salary

$40.17/Hour
