Document Commissioning Services are available at City Hall by appointment. Please contact the Clerk's Office at 705-742-7777 ext. 1820 to book an appointment. We cannot guarantee service unless you have made an appointment with the Clerk's Office.

A Commissioner of Oaths is a person authorized by the Province of Ontario to take oaths or declarations when you sign an affidavit or statutory declaration. The Clerk's Office at City Hall has several Commissioners of Oaths. The document you require signed should state that a Commissioner of Oaths is able to provide the service. Please note that we do not have a Notary Public on staff. 

The City is only able to provide certified true copies of documents issued by the City of Peterborough. If you require certified copies of other documents, please contact the organization that issued the original.

The Commissioner of Oaths will review the documents you wish to have commissioned. We reserve the right to refuse commissioning services of any document. Documents in a language other than English cannot be commissioned.

Documents the City will Commission

The Clerk's Office is able to commission the following documents:

  • Delayed birth registration forms, name change forms
  • Consent letter for traveling with a child (travel document)
  • Insurance Claims
  • Vehicle ownership 
  • OSAP - Ontario Student Assistance Program application forms
  • Invitation letters for a visitor's visa
  • Statutory Declaration in Lieu of Guarantor for Canadian Passport
  • Affidavit waiving vaccinations

Documents the City will not Commission

The Clerk's Office will not commission any of the following documents:

  • Age of majority/Ontario photo ID applications
  • Any documents requiring the signature of a guarantor
  • Court and legal documents related to civil matters
  • Custody, divorce, separation, marriage or cohabitation documents
  • Estate settlement documents
  • Incomplete documents (no information may be added after signing)
  • Real estate documents including rental lease agreements
  • Transfers of money or estates of a deceased person
  • Wills or living wills
  • Office Staff will not sign documents as 'witnesses'

Fees

We charge $26.55 plus HST per signature to commission documents. After the first signature, each additional signature is $5 plus HST. Payment may be made by Visa, Master Card, cash or debit.

Documents required under the Vital Statistics Act (i.e. Name Change, Delayed Statement of Birth) will be commissioned free of charge.

Identification required

In order to have a document commissioned, all parties required to sign the document must appear in person and have valid, government issued photo identification.
