Social Services offers Integrated Case Management and will work in collaboration with employment service provider staff to provide wrap around supports for participants. Staff from Ontario Works (OW), Ontario Disability Support Program (ODSP), Learning and Basic Skills (LBS), and Employment Ontario (EO) will work together through case conferencing to share information on goals, current action plans and steps to assist participants in meeting their goals.

Referrals can be made to Family Doctor/Nurse, physical rehabilitation, clinics or hospital services, mental health programming, addiction support, long-term care, and low cost dental services. |
Supports / Resources

Community supports |
Referrals can be made with child care supports, home care, legal clinics, probation services, post incarceration services and settlement and immigration. |

Life skills |
Referrals can be made to LEAP, literacy and basic skills, educational institutions, settlement and immigration and Employment Ontario. |
